Patient Advocate Foundation helps those with a qualifying serious medical condition navigate the healthcare and insurance systems, providing case management and copay relief as available.

COPAY RELIEF
PAF has separate funds for many different chronic diseases.
If PAF has funding available for your diagnosis, they might be able to help you pay insurance premiums and out-of-pocket costs for medication.
Your treatment must be covered by insurance and you must meet income guidelines to qualify.
You can apply online, call 866-512-3861, or ask your doctor or pharmacist apply for you.
Each disease fund has different rules, so please call with any questions.

CASE MANAGEMENT
Services are very limited, and you may have to call periodically to get connected to a case manager. Case managers do not provide direct financial assistance. Services include:
Help understanding your insurance coverage and resolving insurance issues
Help understanding your options if you are uninsured
Help finding and applying for financial assistance programs
Negotiating payment plans, write-offs, and discounts with healthcare providers
Help finding support groups and understanding mental health coverage options
Help understanding and enrolling in SSI, SSDI, COBRA, FMLA, and employer-based disability
