Description:

VAMC Hampton, the fourth oldest medical center in the VA system, is a 468-bed facility which serves a 15-county area in Eastern Virginia and a 10-county area in Northeastern North Carolina.

At Hampton VAMC we consider it a privilege to serve our nation's heroes by providing quality, compassionate health care.

Homeless Veterans (literal) are to be encouraged to call the homeless coordinator at the medical center.

The VAMC has a full-time Primary Care Clinic for Homeless and at-risk Veterans.

Outreach staff Based at the VAMC reaches out to the community Monday through Thursday every week except on federal holidays.

NOTE:
The Veterans Administration in cooperation with HUD recommends using these questions in the field to determine Veteran status and VA eligibility.

1. When in a call, if the person answers yes to the !€œare you a Veteran!€ or !€œis your spouse a Veteran!€ please ask them these additional eligibility questions.
NOTE: Question one is the assessment question.

2. If yes, were you on active duty (not reserves) after 1980 for at least two years or did you serve in combat? (If on active duty prior to 1980, the two-year requirement does not apply.)

3. If yes, did you have an honorable or general discharge?

Affirmative (YES) answers on the second and third questions can help determine whether a Veteran is eligible for VA services. Communities can help end Veteran homelessness by making sure that veterans are properly identified among people experiencing homelessness.

Refer all persons answering questions two AND three YES to the VAMC for further screening.
