Intensive Community Outreach Services, LLC

Description:

Continuous support for families, community members and other professionals in an effort to curtail undesirable behaviors that hinder the social, personal and educational process of the clients we serve. We share proven effective therapeutic interventions with all stakeholders. We reduce the risk for out of home placement. We, along with parents/guardians will represent the client in school and/or the court to celebrate his/her progress to the goals/objectives of the treatment plan. We will educate all care givers and guardians on the behaviors of the diagnosis as it relates to their child.


ICOS is dedicated to helping teens and adults with mental illness and substance abuse strive for recovery and greater independence.

Eligibility:

IIHS
-Adolescents (ages 5-18)
-At risk of out of home placement
Mental health diagnosis
Behavioral problems at home, school or community
Physical or verbal aggression

MHSS
-Adults (ages 18+)
-Currently take medication or have taken in the past
-Hospitalization in the past for mental illness
-pattern of emotional or behavioral concern
-Current or previous mental health diagnosis
-Need for daily living skills training

SA
-Must of one diagnosis for Substance Related and Addictive Disorders
-Insight to impact of substance use and/or co-occurring disorders on life outcome is poor or limited, yet client has some or limited motivation for treatment engagement.
-Relapse and/or engagement in high risk behaviors and situations are high and may be prevented with multiple weekly treatment services
