Colonial Behavioral Health is governed by a Board of Directors, which comprises appointed representatives from each of the four jurisdictions.

Our service provider team comprises fully qualified psychiatrists, psychologists, social workers, counselors, nurses, case managers and other professionals who provide an array of services to fulfill our mission. We work in partnership with the people we serve and their families, community partner organizations and practitioners, elected officials and many stakeholders.

Intake Process:

Phone to schedule an appointment for an assessment.

Assessment Process
The assessment process begins with a face-to-face meeting with our licensed therapist. Upon completion, recommendations are given to the family and consumer choice is offered with the listing of three area providers that can meet their treatment needs. Once a provider is chosen, all efforts are made to create a smooth transition in setting up appointment dates and times.

Program Fees:

Consumer services are provided on a fee for service basis. Most insurance plans and HMOs are accepted, including Medicaid. Persons without insurance coverage are charged according to ability to pay, based on income and dependents. No one will be denied services because they are unable to pay.

If you do not have insurance and are seeking reduced fees for services, please bring the following documents to your assessment:

Proof of where you live (ex. utility bill, driver's license or photo ID with address, etc.)
One month!€™s paystubs or a 2017 W-2
Food and Shelter form if you live in someone else's home.

Eligibility:

Children and teens ages birth to 18, who have developmental, emotional or behavioral issues are eligible for services. Children at risk of developing emotional or behavioral issues may also receive services.
